  • Complete this form to apply for a non-homestead exemption that is a federal or state agency. If you are a governmental agency that is not federal or state do not use this form, instead use the PTAX-300 form.
• Charitable Exemption Form - Hospital (PDF): PTAX-300-H
  • Complete this form to apply for a hospital non-homestead exemption. "Hospital" means any institution, place, building, buildings on a campus, or other health care facility located in Illinois that is licensed under the Hospital Licensing Act and has a hospital owner.
